What Is Actually In Your Mouthwash?

Most of us buy mouthwash based on the front of the bottle. The colour, the word on the label, whether it says fresh or intense or total.

The useful information is on the back, in a list most people never read.

Here is how to make sense of it.

Mouthwash is a cosmetic product

Worth knowing up front. Everyday mouthwash sold in shops is a cosmetic product, the same category as toothpaste and shampoo. It is there to clean and freshen.

Anything sold to treat a condition is a different thing entirely and it usually comes from a pharmacy counter or your dentist rather than a supermarket shelf. If you have a specific problem going on in your mouth, that is a conversation with your dentist rather than a shopping decision.

What alcohol-free actually means

Alcohol has been used in mouthwash for a long time. It carries flavour and helps ingredients stay mixed.

It is also where that sharp sting comes from. Some people like it, because it feels like something is happening. Plenty of people do not and find it uncomfortable enough that they stop using mouthwash altogether.

Alcohol-free simply means the formula does that job another way. It is a preference rather than a ranking. If the sting is the reason your mouthwash has been sitting unused at the back of a cupboard for a year, alcohol-free is worth trying.

One thing that confuses people.

You may find "benzyl alcohol" in the ingredients list of a product labelled alcohol-free, and assume something has gone wrong. It has not. Alcohol-free refers to ethanol, the drinking kind, which is what causes the sting. Benzyl alcohol is a different molecule used in small amounts as a preservative and it is present in plenty of cosmetics that are correctly described as alcohol-free. Same word, different ingredient.

Fluoride and the number next to it

If a mouthwash contains fluoride, the amount is listed in ppm. That stands for parts per million.

Hydroxyapatite

This one has become much more common on labels in the last few years, so it is worth knowing what it is.

Hydroxyapatite is the mineral that natural tooth enamel is largely made from. In oral care products it deposits onto the surface of the tooth, leaving the outside feeling smoother, and a smoother surface reflects light more evenly, which is why teeth can look brighter after consistent use.

The other names you will spot

Hyaluronic acid.

Common in skincare, increasingly used in oral care too, where it moisturises and cares for the gums and oral mucosa.

Xylitol.

A sugar alternative. You will also find it in sugar-free chewing gum.

Sodium lauryl sulfate, or SLS.

A foaming agent. Some people prefer to avoid it and formulas are widely available without it.

Aqua.

Water. Usually the first ingredient on the list.

How to actually read the list

Ingredients are listed in descending order of quantity, so whatever is first is the largest part of what is in the bottle. Aqua is first on most mouthwashes because most of a mouthwash is water.

One caveat worth knowing, because it changes how much you can read into it. The descending order only holds down to 1%. Below that, ingredients can be listed in any order the manufacturer chooses. So the top of the list tells you something real, and the bottom of the list tells you only that everything there is present at under 1%, not which of them there is most of.

That is still useful. It means an active you care about appearing near the top is meaningful, and it means you should not read anything into the relative position of two ingredients at the bottom.

How to actually use it

Mouthwash is an extra step rather than a replacement for brushing.

The important bit is timing. After you brush, spit out the excess toothpaste but do not rinse. Your toothpaste leaves a concentrated layer of fluoride on your teeth and rinsing straight away washes it off. That includes rinsing with mouthwash.

So use your mouthwash at a different time of day. After lunch works well for a lot of people.

Check your pack too, because instructions vary between products. Some tell you not to rinse with water afterwards, so the ingredients stay on your teeth for longer.
